RPGN
Definition: 50%loss of renal function ในระยะเวลา3เดือน + evidence of glomerular injuryเช่น active urine sedimen (hematuria)
Severity ของโรค relate กับ degree ของ crescent formation ส่วนใหญ่ผู้ป่วยที่มี circumferential crescent > 80% of glomeruli มักมี renal function ที่ไม่ดีและมักไม่ตอบสนองต่อการรักษา ต่างจากผู้ป่วยที่มี crescent <50% และเป็นลักษณะ non circumferential ที่มีอาการน้อยกว่า
Pathogenesis crescent formation
เป็น non specific response ต่อ severe injury ของ glomerular capillary wall ทำให้มี plasma product รวมทั้ง fibrinogen move มาอยู่ใน Bowman’s space ทำให้เกิด fibrin formation เป็นผลให้เกิด influx ของ macrophage and T cell เกิดการหลั่ง proinflammatory cytokine ได้แก่ IL1, TNF alpha ดังนั้นการเกิด crescent จึงพบใน severe glomerular disease อะไรก็ตาม เช่น LN, post infectious GN
การที่มี active inflammation มักตามมาด้วยการเกิด fibrocellular และ fibrous crescent
- Fibroblast growth factor and TGF –B กระตุ้นให้เกิด fibroblast proliferation and collagen deposition ถ้าพบ fibrous crescent มักไม่ตอบสนองต่อการรักษาแล้ว

- Goodpasture’s syndrome
- antiGBM disease
Type 2 immune complex
- post infectious : post strep, visceral abscess
- collagen vascular disease : LN, Henoch Schonlein purpura, mixed cryoglobulinemia
- primary renal disease : IgAN, MPGN, MN, idiopathic
Type 3 pauci-immune vasculitis
- wegener’s granulomatosis
- Churg strauss syndrome
- Microscopic polyangiitis
- Hypersensitivity vasculitis (PTU, penicillamine, hydralazine)
- Idiopathic crescentic GN
Type 4: Double-antibody positive disease — Type 4 has features of both types 1 and 3.
Henoch Schonlein purpura classic triad
1. Purpura 100% ขา ก้น
2. Arthritis 80% มักเป็นที่ข้อเท้า ข้อเข่า ข้อศอก
3. Abdominal pain 62% colicky pain
4. GIB 33%
Wegener’s granulomatosis 2/4
1. Nasal or oral inflammation
2. CXR พบ nodule, fixed infiltrate or cavities
3. Microscopic hematuria > 5/HPF or RBC cast
4. Granulomatous inflammation on biopsy
Churg strauss syndrome 4/6
1. Asthma
2. Eosinophillia> 10%
3. Paranasal sinusitis
4. Pulmonary infiltrates
5. Vasculitis จาก patho ร่วมกับมี extravascular eosinophilia
6. Mononeuritis multiplex or polyneuropathy
Note :
ANCA เป็น Ab ต่อ Ag ที่อยู่ใน granule ของ neutrophil ใน lysosome ของ monocyte โดย indirect IF ใช้ ethanol fixed
c-ANCA : coarse,speckled in cytoplasm
p-ANCA : homogenous IF perinuclear cytoplasm
ELISA ตรวจ Ab ต่อ specific antigen มี 2ชนิด เป็น Ag ที่อยู่ใน granule ของ neutrophil
1. Anti MPO = p ANCA à ส่วนใหญ่ Churg-Strauss
2. Anti PR3 = c ANCA à ส่วนใหญ่ Wegener’s
